INSURANCE & PAYMENT

Please note: Dr. Cantu does not accept any insurance policies and would be considered an “out of network” provider for PPO plans.

Dr. Cantu is not a MediCare provider for psychiatry and has opted out of MediCare. By entering into treatment with Dr. Cantu, you must agree not to submit a claim to MediCare for psychiatric services rendered.

PAYMENT

Payment is due at the time of each appointment. You may pay with cash, personal check or Visa/MasterCard/Discover/AmEx cards.

There will be a fee for any returned/NSF checks.

MISSED APPOINTMENTS & CANCELLATIONS

It is our policy to charge you for missed appointments at the full rate of a normal office visit unless a 24-hour advance notice of cancellation is given.

Cancellation calls must be placed with our office, and not the answering service. Monday appointments must be canceled prior to the office closing on Friday 12:00 pm.

Also, patients arriving more than fifteen (15) minutes late for a scheduled appoint may be charged and asked to reschedule.

Please be sure the office has your most current phone number to reach you for any scheduling changes.

MEDICAL RECORDS

Copies of your medical records are never allowed to leave this office without a signed Authorization Release Form and the approval of your treating practitioner. Our staff members have completed training involving confidentiality and protecting your privacy and will not release any information outside of the practice without your approval.

Texas law provides that a professional may disclose confidential information only to medical or law enforcement personnel if the professional determines that there is a probability of imminent physical injury by the patient to himself/herself or others, or if there is a probability of immediate mental or emotional injury to the patient.

Any fee or charge for copying, mailing, or otherwise complying with an appropriate request for the release of medical records will be billed to the responsible party.

COMMUNICATIONS

Our staff does not communicate via e-mail unless specifically requested by either party under special circumstances.
